Baby Shower Questionnaire

My son lives in another state. I am going to have an out of town Baby Shower for friends and relatives. I would like to create a Baby Questionnaire(Baby advice) for the guests to fill out and attach their picture, so when the mom to be gets the gifts, she will know who sent the gift and get some good baby advice. I need help coming up with questions.. One example is: The Best Way To Quiet A Fussy Baby Is_____________________. I would like to have 10 to 12 questions..
